All fees are due at the time of service.

  • The fee for therapy is based on session duration.

  • We operate under a cash pay model for all services, except for patients using Lyra or Modern Health. This means that we do not bill insurance directly. If you wish to use insurance coverage, we can provide a detailed receipt (superbill) to submit for out-of-network reimbursement with your insurance company.

  • Health insurance policies and reimbursement are between yourself and your health insurance company. All services rendered by Dr. Erika Miley for the benefit of yourself or dependents are charged directly to you, and you are personally responsible for payment in full.

  • Payments may be made with credit/debit/health card. If your payment is declined after your session, you will have 72 hours to remedy payment.

  • If your outstanding balance due to Dr. Erika Miley for treatment becomes Five Hundred Dollars ($500.00) or more, Dr. Erika Miley reserves the right to withhold therapy up to and until such balance is paid in full.

  • If no payment is received, a 4% late fee will be charged after 30 days.  After 60 days of no payment, we will begin collection activities which may include contacting an outside collection agency. 

FEE SCHEDULE

Individual/Family Therapy
Intake Session - $200 per 90 minute session
Individual - $180 per 50-minute session
Couples Therapy - $250 per 90 minute session

Group Therapy
$50-$90 per group session per person

Assessments
Adult ADHD & Autism - $350 per 90 minute session (multiple sessions may be needed to complete assessment)

CANCELLATION POLICY

Regular attendance is essential for your growth in therapy. We understand that life is unpredictable and cancellations will happen. Please cancel ASAP by email.

  1. Sessions cancelled with 24+ hour notice will not be billed.

  2. Please remember that if you do not show for a scheduled session or if you give less than a 24 hour notice, you will be charged the session fee.

  3. If you need to be late for your session, please email a heads up ASAP. If you do not email or join the session within 10 minutes of your appointment, you will be considered a no-show. You will be billed for a full session, and you will need to reschedule. Three no-shows may result in a termination of service.

  4. If a session starts late, it will still end at its scheduled time and the rate will not be adjusted. Clients who are 15+ minutes late for a 50 minute session will need to reschedule.

  5. After multiple cancellations (5+ in a three-month period) for any reason, clients may be subject to a charge of the full rate for that session.
